Mission

The mission of St. Luke Community Clinic is to provide community-based care for people trying to improve their lives, but lacking financial resources necessary to afford adequate medical care.

Believing that citizens deserve access to adequate medical care, our goal is to provide this care through the efforts of volunteer professional and support staff and the contributions of individuals, churches, civic groups and businesses who share the vision.

St. Luke Community Clinic is committed to providing a high level of comprehensive care to clients with chronic health care needs. This goal has required the clinic to change the way care is provided, specifically by focusing on the management of chronic disese in an ongoing program. People with chronic health care problems like diabetes, hypertension or asthma, who comprise over one third of St. Luke's clients, face daily challenges that require life style changes and often include the need for education, a regimen of prescription medications and follow up. This group may be medically stable but have not adopted health care behaviors that limit disease progression and prevent serious complications. For clients who are struggling to adhere to a health management plan, the addition of a Nurse Practitioner has provided a safety net of case management, education, expert clinical care and follow up.
