Eligilibity
St. Luke will provide free medical care and prescription medication for individuals whose total family income is less than 150% of the current Federal Poverty Guidelines and who do not have insurance. St. Luke will make every effort to supply samples of medications and to assist the patient in getting medications through the Pharmaceutical Companies Patient Assistance Programs.
Eligibility will be determined by an application and verification of income. A patient with income higher than 150% of the current Federal Poverty Guidelines. Who has significant finanial hardship due to extenuating circumstances may request services by submitting a fiancial appeal to the Executive Director.
Income may consist of salary and wages, child support payments, alimony, disability benefits from public programs or insurance programs, pension payments, Social Security benefits, Supplementary Security Income payments, Aid to families of Dependent Children (Welfare) payments, Unemployment Compensation, income from trust and court awards.
